We aim to have full transparency in costs & keep it simple.
There are 2 models to choose from – Direct Primary Care Membership and Functional Medicine Membership. The correct model for you depends on your goals – see below for details. If you are unsure about membership, we also offer a single Functional Medicine Consultation appointment detailed below.
Both Models Include:
Your in-office visits, tele-medicine visits, in-office rapid resulted labs (strep, flu, urinalysis, urine pregnancy test, finger blood glucose), procedures we can offer in clinic, text messaging, phone consultations, lab reviews, and emails
Wholesale access (35% off retail value) for recommended professional grade vitamins & supplements – we do not take any profit from this and pass the entire clinician discount to you
*Costs of lab tests are separate and are billed directly from the lab testing facility. Patients may apply their insurance for this.
Direct Primary Care Membership:
$98/month
One Time Enrollment Fee: $120
Initial visit:
90 minutes face to face + 60-120 minutes of pre and post visit case review and personalized plan building
Follow up visits:
30-45 minutes face to face + 60 minutes pre and post visit case review and personalized plan building
Holistic approaches with a focus on supporting the body’s healing will be preferred, while prescription medications are used not as default, but when necessary. Functional work-ups are not included. We recommend the functional model instead for anyone looking to find underlying causes and working through a condition.
The savings we provide our patients with wholesale access (35% off) for professional grade vitamins & supplements as well as labs can easily amount to the cost of the DPC membership – giving you full time primary care for free. These discounts are a courtesy to our patients.
Functional Medicine Membership:
$150/month
One Time Enrollment Fee: $395
Initial visit:
90 minutes face to face + 120-240 minutes of pre and post visit case review and plan building as needed
Follow up visits:
30-45 minutes face to face + 60-120 minutes pre and post visit case review and plan building
In addition to everything offered in the Direct Primary Care Membership, functional approaches to treatment are included with a detailed explanation of functional test results and individualized treatment protocols. We do not perform all functional tests.
We recommend the functional model for anyone looking to find underlying causes and work through a condition through a focus on gut healing, SIBO treatments, and hormonal imbalances. Our functional tests include NMR testing, GI Map (comprehensive stool test), SIBO Lactulose Breath Test, DUTCH (complete hormonal testing), and Organic Acid testing. We have seen a lot of success with patients that invest their efforts to correct underlying imbalances with their digestive health, nutrition, lifestyle, and hormones. Costs of lab tests are separate and are billed directly from the lab testing facility. Patients may apply their insurance for labs.
Functional Medicine Consultation: $550
90 minute face to face + 60 minute pre-visit medical case review + post visit recommendations document
If you are unsure about membership, you can schedule a one time Functional Medicine Consultation appointment.
This virtual appointment includes obtaining a detailed history of your story, reviewing any prior labs/records you provide in advance, and providing our medical recommendations regarding your symptoms, recommended next steps in work ups and potential treatment pathways. This service offers a consultation only and does not include any lab orders, prescriptions, or treatment orders. It is used as a comprehensive evaluation and suggested treatment course that you can take back to your medical providers to implement. If you choose to then join the Functional Membership, 100% of the cost of this consultation can be applied to cover the enrollment fee of the Functional Membership.
**Patients in the Functional Membership Model that have completed functional workups and are looking for maintenance care without ongoing functional assessments will be switched to the lower cost Direct Primary Care Membership.
**Patients in the Direct Primary Care Membership who would like to switch to the Functional Membership Model will be charged the difference in enrollment fees: $275
LABS
Any rapid tests we perform in house are included with the visit.
Any labs that we send out for results will have a separate charge from the lab (we can apply your insurance for these).
We give access to all patients to extremely discounted lab prices. These prices are well below insurance pricing. Patients may use their own insurance for labs or our pricing list.
