• Functional Packages (Non-DPC): This is now open for scheduling
  • Functional packages are all virtual and visits are divided between Dr. Haque and Rachel, NP-C

• State laws require patients to be in Virginia during their telemedicine visits.

• If you have Medicare insurance, please see our section on Medicare.  We would love to see you, but until we have completed the Opt-Out process,  federal law prohibits our treatment for you even if you do not use your Medicare insurance.
