Please Read Before Scheduling:
- DPC Program: Our primary care membership will be accepting new patients again under Roots DPC by NVH with Dr. Haque starting August 26th 2024. At that time, Dr. Haque will begin scheduling free consultation calls for those considering membership.
- Current Members: online scheduling is limited to tele-visits. For in-person appointments, please call or message us directly.
- Functional Packages (Non-DPC): This is now open for scheduling
- Functional packages are all virtual and visits are divided between Dr. Haque and Rachel, NP-C
- State laws require patients to be in Virginia during their telemedicine visits.
- Please read the details of what is included in packages/pricing before scheduling – see our online pages dedicated to the description of each package.
- Medicare insurance: We are now seeing patients who have Medicare but we do not accept Medicare insurance. Please see our section on Medicare. Medicare patients may schedule with Dr. Haque now and with Rachel starting October 2024.
