DPC PROGRAM: Our primary care membership is not accepting new patients currently
FUNCTIONAL PACKAGES (NON DPC): We are accepting new patients. These are fully virtual and visits are divided between Dr. Haque & Rachel, NP
THE MENOPAUSE CLINIC: We are accepting new patients
State laws require patients to be in Virginia during their telemedicine visits

Medicare insurance – We are seeing patients who have Medicare, but we do not accept Medicare insurance. Medicare requires an Opt Out Agreement in which patients (and we) cannot request/submit for reimbursement of visits. You are still able to use Medicare for any prescriptions, imaging, lab orders, or referrals we provide for you. This does not affect your use of Medicare outside of our practice.
Appointment confirmations will be sent to your email address. If you do not receive a confirmation, please check your spam or junk folder.
Please complete your Charm EHR questionnaires at least one day before your scheduled visit. Failure to submit them on time will result in cancellation of your appointment.
